I, the undersigned, hereby certify that I am the legal parent/guardian of the above-named child and consent to their participation in all activities associated with Swish Camp. I understand the nature of these activities and acknowledge that while reasonable precautions will be taken to ensure safety, participation in physical activities may involve certain inherent risks.

I, release, waive, and discharge Swish Camp, its directors, officers, employees, and agents from any and all liability, claims, or demands arising from my child’s participation in camp activities, except in cases of gross negligence or willful misconduct.

In the event of an emergency, I authorize Swish Camp staff to arrange for necessary medical treatment for my child if I cannot be reached. I accept financial responsibility for any medical expenses incurred.

By opting into Carpool Connections, I acknowledge that Swish Camp is not responsible for any arrangements, actions, or outcomes resulting from carpooling with other families. Participation in this program is voluntary and facilitated solely to connect interested families.
